I give my consent to Poytaxt Bank JSC (the Bank) to process all my data specified in the application form by any means, including by third parties, including reproduction, electronic copying, depersonalization, blocking, destruction, as well as the above processing of my other data obtained as a result of their processing, for the purpose of:

• entering into an contract(s) with the Bank;
• establishing the Bank's data information systems;
• insurance of my life/health/property and other insurance carried out with the assistance of the Bank or in favor of the Bank and/or in connection with the conclusion of the contract;
• as well as for any other purposes, directly or indirectly related to servicing in the Bank and offering other products of the Bank, and sending me information about new products and services of the Bank and/or its counterparties.

I give my consent to the Bank's counterparties to process all my data in the possession/access of the Bank and/or the Bank's counterparties, including the processing of data on subscribers and communication services provided to them (in case the Bank's counterparty is a communication operator) in order to assess the probability of my solvency in the future in order to make a decision on granting microloans, loans, opening cards, etc.

I give my consent for the Bank to provide all the information about me in the amount, in the manner and on the terms necessary for my identification and carrying out the appropriate assessment of my solvency, risk assessment, to the State Register of Credit Information under the Central Bank of Uzbekistan, to all credit bureaus included in the State Register of Credit Bureaus, the State Unitary Enterprise “Pledge Register” under the Central Bank of Uzbekistan, the Bureau of Compulsory Enforcement under the General Prosecutor's Office of the Republic of Uzbekistan, other governmental and non-governmental agencies and organizations, as well as to obtain credit history, information on pledged property and encumbrances, information on debts, seizures and other data from the abovementioned bureaus and agencies for the conclusion of the contract(s) and during the term of the contract(s).

The Bank shall have the right to suspend consideration of the application in case of necessity to provide additional information (documents) on the part of the applicant, in cases stipulated by the legislation of the Republic of Uzbekistan and local acts of the Bank.

The Bank shall have the right to refuse to consider an application if incomplete or unreliable (false) data are indicated therein, as well as in cases stipulated by the legislation of the Republic of Uzbekistan.
